How should I decide whether to map a pedestrian way in a housing estate as a footpath or as a pedestrian street? The examples that I have in mind are areas where houses have their front doors facing on to a pedestrian area (grass with asphalt paths), with separate road access to the rear of the houses.

The reply is in the title. If you consider the pedestrian way as "The Street" , then I would suggest "highway=pedestrian" + "name=name_of_the_street" (adding "bicycle=yes|no" can clarify the public access for bikers crossing the area). The road access in rear side is tagged with "highway=service" + "service=driveway|alley".
Keep "highway=footway" or "higway=path" for minor paths.

Reading your description, I don't think that highway=pedestrian would be appropriated. Pedestrian area is generally for wide area in urban area. I will recommand highway=path or highway=footway if bicycles are forbidden.
