I was planning to add buildings in a small residential area when I spotted an abandoned building that advertises the services of a doctor (who supposedly passed away years ago). How do I tag it, other than the mandatory building=yes?

You could use building=abandoned or building=disused, whatever fits more (probably the first one makes more sense). These are not very common tags, but have already 27 and 9 usages according to tagstat.
