What to name bus stop if two agencies use the same stop but name them differently?

In York Region Transit, for example, stops are named "X Street/Y Street". However, on Toronto Transit Commission buses, they are usually named "X Street At Y Street". Sometimes the two agencies use the same stops, so which one should I name the bus stop?

In general use the name as shown on the sign ("the flag") at the bus stop. If this does not carry a name use your discretion (I'd regard the two options above as more or less equivalent). The tag unsigned=yes can be useful in such situations, implying the source lies elsewhere (e.g., in announcements on the bus).

You can also use alt_name for one of the alternatives.
