Why aren't lakes and rivers shown at low zoom levels in the default map style, especially at zoom level 5? This is somewhat confusing especially where roads are shown but not some water forms.

That is a design decision by the group that maintains the style you will need to discuss that with them directly: https://github.com/gravitystorm/openstreetmap-carto/issues

This issue is already discussed here: https://github.com/gravitystorm/openstreetmap-carto/issues/1604 . Looks like the main reason is performance.
