I have found two resources on the wiki that describe different ways for adding stop signs: Key:stop and Tag:highway=stop. The key:stop page has not been updated since 2010, so I am assuming that the highway=stop method is the preferred method going forward, but I am still confused by exactly how to place stop signs. From what I gather, it is necessary to add new nodes on the roads leading up to the intersection, and tag each new node with highway=stop and stop:direction=[forward|backward]. This would mean that for a four-way-stop intersection, with single-way roads, four new nodes would have to be added, two marked with stop:direction=forward, and two marked with stop:direction=backward. Note that mapping individual signs is not really useful. Instead you should map the resulting restrictions, permissions and conditions (there are different page versions for various countries).
