I'd like to add a 198 metre high chimney (copper smelter chimney), but I can't see how to get the diameter of the circle to decrease over the length of the chimney.

Hi take the figuration at the bottom of the chimney, like it happens to all buildings. For instance a balcony is only present if its over a way with a tag covered. Take a look at these Wiki site http://wiki.openstreetmap.org/wiki/Buildings .
