Can I manually add potholes in a road? I have tried various apps to do that, but not able to map it clearly.

We do not usually map individual potholes (for laughs, consult TagInfo on potholes). There's a couple of web sites where you can report a pothole to be fixed, like Fix My Street in the UK or similar offerings elsewhere.

Hi what kind of road are you talking about, paved or unpaved, path, track or motorway ? Consider using http://wiki.openstreetmap.org/wiki/Key:smoothness for a stretch of a way as long as potholes are present to describe to state of the road. You’ll have to cut the way into stretches first, on the spot or elsewhere.

I'd also go with smoothness, to denote really differing road conditions. But that is not mapping single potholes, just the overall state of the road. So, to combine Frederik and Hendrik: Don't map individual potholes, map the overall smoothness of the road.
