When counting lanes, do you count total lanes, or lanes in each direction? |

If you draw for each direction a seperate way you count only the lanes in that direction. If you draw one way for both directions you count the lanes for both directions. Do restricted lanes count? http://help.openstreetmap.org/questions/2339/restricted-lanes-with-lanes-tag

Also, how would reversible lanes come into play? BC-99 from Vancouver city center to North Vancouver, and BC-99 through the George Massey Tunnel come to mind.

Well roads with reversible lanes still only have lanes=# number of lanes, and those roads shouldn't be mapped with more than one way (since they have no fixed separation). If you want to tag reversible lanes you will have to find some suitable tag.

The small amount of times I've tagged lanes, I've used the Some tiny roads with "passing points" are really 3
Are you mixing up lanes:left and lanes:right with lanes:forward and lanes:backward? Please read the lanes wiki page again.
