At some point in the past the node for the state of Maryland wasn't correctly linked to the Maryland relation which meant that the boundary of Maryland wasn't well defined.

This has since been fixed but some of the data hadn't been reindexed. I've forced a manual update and it has resolved the issue.

There's no address.state field though: http://nominatim.openstreetmap.org/search/washington%20dc?format=json&addressdetails=1

Since every U.S. city belongs to a state, would it be consistent to also provide address.state = "District of Columbia"?

From wikipedia:

As permitted by the U.S. Constitution, the District is under the exclusive jurisdiction of the United States Congress and is therefore not a part of any U.S. state.

It is in the county of "District of Columbia".
