I don't see an icon for bike/ped under or over crossings

Here's a couple at a college in Minneapolis: http://osm.org/go/T~ffDUQZW-- Many more examples in the downtown area to the NW (although not all of those have the bridge and layer tags).

Just tag the cycleway/footway as usual, and add a "tunnel/bridge=yes, layer=1/-1".

I don't think we need icon for them as they show on the map ok. This is a tunnel http://www.openstreetmap.org/?lat=52.149151&lon=0.12454&zoom=18&layers=M and this is a bridge http://www.openstreetmap.org/?lat=52.156688&lon=0.135505&zoom=18&layers=M
you can open the editor to see what has been done.
